Another major determining factor in the effect of Thursday's decision is how states with large numbers of poor and moderate income residents -- but often politically conservative legislatures and governors -- respond to the federal government's call for greatly expanded Medicaid access, said Allison Hoffman, a lawyer and professor at the University of California Los Angles School of Law who specializes in health policy.
