I have just received an e-mail informing me that the pdf I previously posted is not the latest version of the envelope that was actually sent out.  The version that was sent out did not have the diagonal about U.S. Citizens.  I am told that is the only difference.  As I said earlier, I have never seen the actual envelope, so I cannot say for sure.  But someone working with Kaptain is looking for the actual envelope that was sent out, and when I receive it, I will post it.  Sorry for the confusion.
